2026-27 Pre-Participation Medical Requirements

Prior to any physical activity with a Fitchburg State University athletic team, student-athletes must complete all medical requirements as outlined on our SportsWare website and complete a SWAY concussion baseline  exam. SportsWare is best accessed on a laptop or desktop computer using Google Chrome as the browser. 

Only student-athletes who are new to the program need to complete final steps at the end of this document.. 

To access and complete SportsWare please read and follow these instructions: 

1. Got to www.swol123.net 

2. Enter your FSU student email address (i.e. jdoe123@fitchburgstate.student.edu) into the email box 

3. Enter your previous SportsWare password or create a new password by clicking “Reset Password”. This will send an email with a link to create your new password to your student email account. New student-athletes will need to create a password their first time signing in. If the student-athlete is under 18, please contact us as your parent will need to complete the forms.  (Do not hit “Join SportsWare”. There is no School ID.)

4. Read and electronically sign all outstanding “Forms”

5. Under "Status" Click into each Incomplete section and complete the required information. 

a) Complete all Athlete Information marked with a red asterisk and upload pictures of both sides of your insurance card. 

b) Complete all Medical History questions with a "Yes" or "No". Provide details to any questions which you answer "Yes" in the comments section 

c) Complete the PH-Q9 Assessment 

6. Review Notices and Handbooks

7. Returning student-athletes may be required to submit additional documentation/complete testing related to medical conditions from the previous year prior to participation

NEW STUDENT-ATHLETES/TRANSFERS 

1. Submit your Sickle Cell Solubility test results to us by uploading them to the attachments section in SportsWare or faxing directly to our office at the number below. 

2. Complete your SWAY Baseline exam using the instructions and code found in Notices and Handbooks 

3. Once the above steps are completed, we will schedule you for your physical screening with our team physician.
